<p>Union brass&#32&#104&#97ve so far <s></s>decided not to decertify like their coun&#116&#101&#114parts with the National Football Leag<input id="apps" type="hidden" />ue, although &#116&#104&#101 basketball players say they haven&#8217;t ruled out that option.</p>
<p>The &#111&#119&#110ers locked <s></s>out the players on July 1 when they fai&#108&#101&#100 to reach a deal before the expiration<input id="stats" type="hidden" /> of the old &#99&#111&#108lective bargaining agreement. <p>Team owners and players have been&#32&#104&#97ggling over how to divide $3.8 billion <input type="hidden" />in annual r&#101&#118&#101nues and also over salary cap issues.</p>
<p>The main stu&#109&#98&#108ing block has<input id="apps" type="hidden" /> been the issue of how to share baske&#116&#98&#97ll-related income. The owners want a 50-<input id="phpint" type="hidden" />50 share o&#102&#32&#114evenues while the players refuse to go below 53 pe&#114&#99&#101nt.</p>
<p>The gap in<input type="hidden" /> their positions has been so wide it&#32&#99&#111uld now jeopardize the entire 2011-2012 c<input id="stats" type="hidden" />ampaign, &#115&#97&#121s Fisher.</p>
<p>&#8220;This is not where we choose to be,&#8221; Fisher said. &#8220;We&#8217;re not at a place where a fair deal can be&#32&#114&#101ached with the <input id="stats" type="hidden" />NBA.&#8221;</p>

<p>Pain is defined as chronic when it persists a&#102&#116&#101r an injury or illness has oth<input type="hidden" />erwise healed, or wh&#101&#110&#32it lasts three months or longer. The experience of&#32&#112&#97in c<input id="tracker" type="hidden" />an vary dramatically, depending in part on whe&#116&#104&#101r it is affecting bones, muscle<s></s>s, nerves, joints o&#114&#32&#115kin. Untreated pain can itself become a disease wh&#101&#110&#32the b<s></s>rain wrongly signals agony when there is no n&#101&#119&#32injury or discernable other caus<s></s>e. Fibromyalgia—&#32&#97&#32disease in which pain in joints, muscles and other&#32&#115&#111ft tis<s></s>sues is the primary symptom—is believed to&#32&#98&#101 linked to incorrect signaling in<input id="counter" type="hidden" /> the brain’s pa&#105&#110&#32regions.</p>
<p>Finding a Doctor</p>

<p>Dr. &#80&#97&#117l Christo, director of the multidisciplinary pain &#102&#101&#108lowship pro<input type="hidden" />gram at Johns Hopkins School of Medicin&#101&#44&#32also suggests looking for someone who <input id="tracker" type="hidden" />has complete&#100&#32&#97t least a year-long certification in pain manageme&#110&#116&#46 This inform<s></s>ation can usually be obtained on the d&#111&#99&#116or’s website or by asking about his o<s></s>r her quali&#102&#105&#99ations.</p>
<p>Comprehensive Treatment</p>
<p>Experts agree that&#32&#99&#111mprehensive c<s></s>are—which can involve medications, &#101&#120&#101rcise, psychological therapy, massage, p<input type="hidden" />hysical th&#101&#114&#97py, injections and complementary treatments, depen&#100&#105&#110g on the patie<s></s>nt and condition—is essential.</p>
<p>“&#84&#104&#101 reason we now call chronic pain an illne<input id="counter" type="hidden" />ss is tha&#116&#32&#119e recognize that it is more than just a sensation &#105&#110&#32the body,” Po<input id="apps" type="hidden" />rtenoy says, “It affects your abi&#108&#105&#116y to function as a human being, your relat<input id="phpint" type="hidden" />ionships&#44&#32&#121our ability to be productive, to think straight.&#157</p>&#10<p>&#85nfortunately, be<input id="apps" type="hidden" />cause they have so often been dism&#105&#115&#115ed as having a problem that’s “all in y<s></s>our hea&#100&#44&#226 many people with chronic pain resist considerin&#103&#32&#116alk therapy as a <input id="apps" type="hidden" />part of treatment.  </p>
<p>“A lot of &#112&#101&#111ple have the misconception that what I’m t<input id="tracker" type="hidden" />elling&#32&#116&#104em [when recommending therapy] is that their pain &#105&#115&#32a figment of their<input id="stats" type="hidden" /> imagination,” Christo says. &#128&#156&#84hat’s not what we mean. Pain has such an em<input id="tracker" type="hidden" />otion&#97&#108&#32component and psychotherapy is extremely useful in&#32&#116&#101rms of helping pati<input id="counter" type="hidden" />ents reorganize and rethink how&#32&#116&#104ey interpret it and how it affects their lives<input id="apps" type="hidden" />.”</p>&#10<p>&#83&#97ys Shaffer, “Pain encompasses the entire person.&#32&#73&#116’s not just in you<s></s>r leg or back. It encompasses &#116&#104&#101 entire being of who you are and what you can d<input id="counter" type="hidden" />o a&#110&#100&#32don’t do. So physically, mentally psychologicall&#121&#58&#32you have to take care<input id="tracker" type="hidden" /> of all of those things.”</p>
<p>T&#104&#101&#32Opioid Question</p>
<p>Although drugs like aspirin, ibu<input id="apps" type="hidden" />pr&#111&#102&#101n and even some antidepressants can help relieve p&#97&#105&#110, the most effective m<input id="phpint" type="hidden" />edications for most severe p&#97&#105&#110 remain the opioids, like Oxycontin and morphine.<input id="phpint" type="hidden" /> &#32&#66&#111th doctors and patients tend to fear these drugs b&#101&#99&#97use of concerns about a<input id="stats" type="hidden" />ddiction and overdose.  </p>
<p>Ho&#119&#101&#118er, of patients without a prior history of addicti<input id="tracker" type="hidden" />&#111&#110&#44 less than 3 percent of patients who take opioids &#114&#101&#103ularly for pain will bec<s></s>ome addicted to the drugs,&#32&#97&#99cording to a Cochrane review of studies.  Opioids &#97<s></s>&#114&#101 currently under a cloud because of a sharp rise i&#110&#32&#111verdose death and addicti<input id="stats" type="hidden" />on, mostly resulting from&#32&#109&#105suse by people who aren’t pain patients.   The m&#97&#106<input type="hidden" />&#111rity of overdoses occur in people who abuse the dr&#117&#103&#115 along with alcohol and de<s></s>pressants like benzodiaz&#101&#112&#105nes (for example, Xanax).</p>
<p>Virtually everyone who t&#97&#107&#101<input id="stats" type="hidden" />s opioids on a daily basis will become physically &#100&#101&#112endent, however:  They will<input id="phpint" type="hidden" /> suffer withdrawal if t&#104&#101&#32drugs are not slowly tapered.  But that is not the&#32&#115&#97m<input id="stats" type="hidden" />e as addiction, which is defined by craving, nega&#116&#105&#118e consequences, reduced abil<input id="tracker" type="hidden" />ity to function and co&#109&#112&#117lsive drug-related behavior. </p>
<p>Kelly Young avoided &#111&#112&#105oi<input id="stats" type="hidden" />ds for years, relying on high doses of ibuprofen&#32&#40&#65dvil) and similar drugs.  But<input type="hidden" /> when the pain became&#32&#101&#120cruciating, her doctor suggested she try an opioid&#46&#32&#226I<input id="tracker" type="hidden" /> was afraid of side effects,” she says. “On&#101&#32&#110ight it was really bad so I to<input id="phpint" type="hidden" />ok it.”  At first,&#32&#115&#104e felt severe dizziness.  “But in 30 minutes, th&#101&#32&#112ain <input id="counter" type="hidden" />started going away and I thought, ‘This is a&#109&#97&#122ing, this is the first time in <input id="phpint" type="hidden" />4-5 years that I’&#118&#101&#32been without pain,’” she says.</p>
<p>To reduce the d&#105&#122&#122iness<input id="counter" type="hidden" />, she cut the dose, starting with a liquid us&#117&#97&#108ly given to children so that she<s></s> could find a leve&#108&#32&#116hat allowed her to be most comfortable.  Neither Y&#111&#117&#110g nor <input id="phpint" type="hidden" />Shaffer, who also manages her pain with opio&#105&#100&#115, has ever developed addiction.</p>
<p>T<input id="stats" type="hidden" />wo-Way Trust</p>
<p>Beca&#117&#115&#101 doctors can lose their licenses or go to prison i&#102&#32&#116hey don<input id="phpint" type="hidden" />’t detect addicts who fake pain, patients&#32&#102&#105nd themselves in a difficult posit<input id="tracker" type="hidden" />ion when they wa&#110&#116&#32to discuss opioid medications.  Asking for a drug &#98&#121&#32name, fo<input id="counter" type="hidden" />r example, which might be fine with other &#99&#111&#110ditions, is seen as a “red flag.<input id="tracker" type="hidden" />  </p>
<p>“When yo&#117&#32&#105nitially go to an appointment, you don’t want to&#32&#103&#111 in there<input id="phpint" type="hidden" /> saying I need medication; that’s the worst thing&#32&#121&#111u can do,” Shaffer says. “You wa<s></s>nt to ask for &#114&#101&#108ief.  Explain to the health care provider, ‘This&#32&#105&#115 my life. <input id="stats" type="hidden" />I can’t get out of bed. I can’t do l&#97&#117&#110dry. I can’t pick up my child.  I n<input id="phpint" type="hidden" />eed quality o&#102&#32&#108ife, that’s why I’m here.’”</p>
<p>Shaffer adds, &#226&#128&#156You have to<s></s> be upfront and honest and build that r&#101&#108&#97tionship of trust with your doctor.”</p><input id="phpint" type="hidden" />
<p>Shaffer als&#111&#32&#110otes that it is the patients’ responsibility to &#115&#116&#111re opioids i<input id="tracker" type="hidden" />n a locked box safely:  Many people wh&#111&#32&#97buse and overdose on these medications <input type="hidden" />get them fr&#111&#109&#32friends and relatives who do not secure them.</p>
<p>Acce&#112&#116&#97nce and Hope</p>
<p><input id="stats" type="hidden" />Shaffer and Young both recommend a mi&#120&#32&#111f realism, mutual support and fighting s<input id="tracker" type="hidden" />pirit when&#32&#105&#116 comes to facing pain.  Young runs her Rheumatoid &#65&#114&#116hritis Warrior<input type="hidden" /> website and Shaffer moderates onlin&#101&#32&#100iscussion groups for people in pain at th<input type="hidden" />e America&#110&#32&#80ain Foundation site.  Experts agree that support f&#114&#111&#109 family, friend<input id="stats" type="hidden" />s and people facing similar problem&#115&#226&#128so long as there’s some type of social <input id="tracker" type="hidden" />support&#128&#148&#105s essential.</p>
<p>“You have to accept what your life &#105&#115&#32going to be, but<input id="counter" type="hidden" /> you don’t have to give up,” s&#97&#121&#115 Shaffer, “OK, yes I have pain but that p<input id="counter" type="hidden" />ain doe&#115&#110&#226t own me or define who I am today.”</p>

<p>Virtually everyone who t&#97&#107&#101<input id="apps" type="hidden" />s opioids on a daily basis will become physically &#100&#101&#112endent, however:  They will<s></s> suffer withdrawal if t&#104&#101&#32drugs are not slowly tapered.  But that is not the&#32&#115&#97m<input id="tracker" type="hidden" />e as addiction, which is defined by craving, nega&#116&#105&#118e consequences, reduced abil<input id="stats" type="hidden" />ity to function and co&#109&#112&#117lsive drug-related behavior. </p>
<p>Kelly Young avoided &#111&#112&#105oi<s></s>ds for years, relying on high doses of ibuprofen&#32&#40&#65dvil) and similar drugs. <p>To reduce the d&#105&#122&#122iness<input id="apps" type="hidden" />, she cut the dose, starting with a liquid us&#117&#97&#108ly given to children so that she<input id="phpint" type="hidden" /> could find a leve&#108&#32&#116hat allowed her to be most comfortable.  Neither Y&#111&#117&#110g nor <input id="apps" type="hidden" />Shaffer, who also manages her pain with opio&#105&#100&#115, has ever developed addiction.</p>

<p>Beca&#117&#115&#101 doctors can lose their licenses or go to prison i&#102&#32&#116hey don<input type="hidden" />’t detect addicts who fake pain, patients&#32&#102&#105nd themselves in a difficult posit<input type="hidden" />ion when they wa&#110&#116&#32to discuss opioid medications.  Asking for a drug &#98&#121&#32name, fo<input id="apps" type="hidden" />r example, which might be fine with other &#99&#111&#110ditions, is seen as a “red flag.<input id="phpint" type="hidden" />  </p>
<p>“When yo&#117&#32&#105nitially go to an appointment, you don’t want to&#32&#103&#111 in there<input id="stats" type="hidden" /> saying I need medication; that’s the worst thing&#32&#121&#111u can do,” Shaffer says. “You wa<input id="counter" type="hidden" />nt to ask for &#114&#101&#108ief.  Explain to the health care provider, ‘This&#32&#105&#115 my life. <input id="tracker" type="hidden" />I can’t get out of bed. I can’t do l&#97&#117&#110dry. I can’t pick up my child.  I n<input id="tracker" type="hidden" />eed quality o&#102&#32&#108ife, that’s why I’m here.’”</p>
<p>Shaffer adds, &#226&#128&#156You have to<s></s> be upfront and honest and build that r&#101&#108&#97tionship of trust with your doctor.”</p><input id="tracker" type="hidden" />
<p>Shaffer als&#111&#32&#110otes that it is the patients’ responsibility to &#115&#116&#111re opioids i<input id="counter" type="hidden" />n a locked box safely:  Many people wh&#111&#32&#97buse and overdose on these medications <input id="apps" type="hidden" />get them fr&#111&#109&#32friends and relatives who do not secure them.</p>
<p>Acce&#112&#116&#97nce and Hope</p>
<p><input id="tracker" type="hidden" />Shaffer and Young both recommend a mi&#120&#32&#111f realism, mutual support and fighting s<input id="stats" type="hidden" />pirit when&#32&#105&#116 comes to facing pain.  Young runs her Rheumatoid &#65&#114&#116hritis Warrior<s></s> website and Shaffer moderates onlin&#101&#32&#100iscussion groups for people in pain at th<input type="hidden" />e America&#110&#32&#80ain Foundation site.  Experts agree that support f&#114&#111&#109 family, friend<input id="stats" type="hidden" />s and people facing similar problem&#115&#226&#128so long as there’s some type of social <input id="tracker" type="hidden" />support&#128&#148&#105s essential.</p>
<p>“You have to accept what your life &#105&#115&#32going to be, but<input id="counter" type="hidden" /> you don’t have to give up,” s&#97&#121&#115 Shaffer, “OK, yes I have pain but that p<input id="tracker" type="hidden" />ain doe&#115&#110&#226t own me or define who I am today.”</p>
